Cochlear Implant Surgery

Cochlear implant is a surgically implanted electronic device that provides a sense of sound to a person who is profoundly deaf. The cochlear implant is also referred to as a bionic ear.

 

Cochlear implants can restore hearing in patients who suffer deafness due to loss of sensory hair cells in their cochlea. By undergoing a cochlear implant surgery a patient restores sufficient hearing to allow unaided understanding of speech in a quiet background, but it is much less rich than natural hearing, and also offers very limited appreciation of musical melody, or speech understanding in noisy environments.

Since 2008 India has witnessed a sudden increase in number of patients coming to India for Cochlear implant surgery. Patients with hearing defects come to India mainly from the Middle East countries like Oman and Iran and from a few Western world countries. But before undergoing an implant surgery a patient is screened for eligibility. There are a number of factors that determine the degree of success to expect from the operation and the device. Cochlear implant centers in India determine implant eligibility on an individual basis and take into account a person's hearing history, cause of hearing loss, amount of residual hearing, speech recognition ability, health status, and family commitment to aural habilitation/rehabilitation. An eligible candidate is described as having severe to profound sensorineural hearing impairment in both ears and having lived at least a short amount of time without hearing (approximately 70+ decibel hearing loss, on average). Also it is taken care in India that the patient is having no medical reason to avoid surgery.

 

The device is surgically implanted under a general anesthetic, and the operation usually takes from 3 to 5 hours. First a small area of the scalp directly behind the ear is shaved and cleaned. Then a small incision is made in the skin just behind the ear and the surgeon drills into the mastoid bone and the inner ear where the electrode array is inserted into the cochlea. The patient stays in the hospital for 1 to 2 days.


Risks involved

Just like every medical procedure, the surgery involves a certain amount of risk; in this case, the risks include skin infection, onset of (or change in) tinnitus, damage to the vestibular system, and damage to facial nerves that can cause muscle weakness and impaired facial sensation. There is also the risk of device failure, usually where the incision does not heal properly. This occurs in 2% of cases and the device must be removed. The operation also destroys any residual hearing the patient may have in the implanted ear; as a result, some doctors advise single-ear implantation, saving the other ear in case a biological treatment becomes available in the future.
